Dr Nahal Habibi (known as Dr Nelly) respects your privacy.

Information you provide

The discovery enquiry form collects your name, email, mobile number, preferred contact method, brief answers about the support you seek and any current support, and your availability. You choose how much to share in your brief answers. Please leave medical records, test results and detailed health information for your appointment.

Why it is collected

I use your details to respond to your enquiry, understand your initial priorities and arrange a discovery conversation. Submission is voluntary. Without contact details, I may be unable to respond. Submitting the form does not subscribe you to marketing emails.

Delivery and storage

Your enquiry is processed through an email service and sent to me. The website does not save your answers in a client database. The delivery service temporarily records a request reference to reduce duplicate messages and keeps daily submission counts. Email and website providers may also process technical information needed to operate and protect their services.

Cloud services may process information outside Australia. Google operates infrastructure in multiple countries, including the United States. Provider locations and practices can change. You can contact me with questions about the services used to handle your enquiry.

Access, sharing and security

I use your enquiry to communicate with you. I do not sell your details or send your form answers to Google Ads. Relevant service providers process information to deliver and operate these services. Any sharing with another practitioner will be discussed with you, except where disclosure is required or permitted by law.

Reasonable safeguards are used to protect enquiry information. No online service can guarantee complete security. Information is retained only as needed for the enquiry, any resulting services and applicable legal obligations, and is deleted or de-identified when no longer required.

Optional cookies

With your permission, optional cookies help us improve our business. Google may receive technical information through these cookies; your enquiry form answers and contact details are not sent to Google.

Your choices and privacy concerns

You can leave personal details for the discovery conversation. To request access to or correction of information, ask about deletion, or make a privacy complaint, contact contact@drnahalhabibi.com.au. I will consider your request, explain any applicable limits and respond as soon as practicable. If a privacy complaint is not resolved, you can contact the Office of the Australian Information Commissioner.
